Expiration Date of E-Coupon:

1. The E-Coupon is valid until April 30, 2027.
2. If the E-Coupon expires, the hospital reserves the right to deny service, and the E-Coupon cannot be extended or
   refunded.
3. This package is available for purchase only until April 30, 2027.
